Application Process
Applications must be submitted in person, at the Cook County Bureau of Human Resources located at 118 North Clark Street, Suite 834
Chicago, Illinois M-F/ 8:30 am until 4:30, therefore cannot currently be accepted online or via mail, fax or email. To speed the application process, you may wish to download the Cook County Employment Application Form prior to visiting our office. Forms may be completed in advance, but should not be signed or dated until they are presented in the Human Resources office.
- Applicants must present a photo ID for verification purposes.
- Applications must be completed in full. Copies of all required diplomas, degrees and licenses must be provided at the time of application.
- All applicants must meet minimum stated qualifications in order to be considered for interview.
Out-of-town job seekers may submit a letter of interest with resume, but will be required to complete the application process in person prior to being interviewed. Letters of interest must specify the job posting number for the position you are seeking. The posting number is located in the upper right corner of each posting, beginning with "DPH" followed by a numeric sequence. Resumes submitted without reference to a current posting number will not be kept on file.
For internship opportunities at the Cook County Department of Public Health (CCDPH) please submit the following:
- A transmittal letter and resume
- A copy of your school’s internship requirements including timeframes
- A proposal of what you can offer or what you hope to experience at CCDPH
